Lakeview provides neurological and behavioral rehabilitation and supported community programs in a non-institutional setting.

Admission Criteria

• Neurological and behavioral diagnosis
• Medically stable
• 6 years of age or older
• Cognitive level of 4 or above on the Rancho Los Amigos Scale
• Potential to improve functional status and benefit from treatment.

Referral Process
A referral can be made by an individual, family member, case manager or other professional.

1. Call the admissions department where initial intake information is obtained.
2. Send pertinent medical records for clinical review for admission.
3. Schedule a tour of Lakeview and/or have client evaluated at present site.
4. Evaluation process, treatment plan, fee projections and recommendations will be developed and sent to the appropriate parties.
5. Secure funding for treatment at Lakeview
6. Tentative admission date is established.
7. Admission papers are completed and the client is admitted.
   

Post Acute Care
Our continuum of post acute care includes but is not limited to specialized services for individuals with neurological/behavioral impairments secondary to the following:

Traumatic Brain Injury (TBI) Cerebral Vascular Accident (stroke) Pervasive Developmental Disorder Alcohol or Substance Abuse related Meningitis Encephalitis Brain Tumor Tourettes Syndrome Huntington's Disease Mild to Moderate Mental Retardation and Developmental Disorders

Autism Tic Disorders Multiple Sclerosis Cerebral Aneurysm Fetal Alcohol Syndrome Seizure Disorder Birth trauma Cerebral Palsy Hydrocephalus Anoxia/Hypoxia ADD/ADHD/OC/CD Impulse Control Disorders Asperger's Syndrome
